Explore Our Facility Care Plan

Our dedicated Lone Tree team develops a care plan for each of our residents. A care plan is a personalized, comprehensive document that outlines the individual’s specific medical, daily living, social needs, and their history to get a better understanding of the person they are to this day. Having this information is beneficial to staff to building relationships and create strategies to better care for them. It serves as a roadmap for care to ensure the resident attains or maintains their highest practicable physical, mental, and psychosocial well-being.

Together we developed this facility care plan to display the Lone Tree’s homelike environment and recognize the departments and services that are continuously maintained with the residents’ best interests in mind.
